What is GORD?
Gastro-oesophageal reflux disease

Mouth
Tongue
Saliva

Tonsils
Teeth
Oesophagus
Early digestion – mouth
Taste – tongue
Saliva – enzymes, antibacterial, pH control, taste, lubrication
Palate – press against to aid digestion
Tonsils – immunity, lymph
Teeth – digestion
Oesophagus = passage
What is oesophagus made of?
What supplied by?
Upper 1/3- skeletal muscle
- superior laryngeal and recurrent laryngeal nerve
Lower 2/3- smooth muscle
- enteric nervous system
What process moves food?
Peristaltic waves
What is at entrance to stomach?
Cardiac sphincter- control entry into cardia
State cause GORD?
Poor oesophageal motility
Cardiac sphincter wakened- less pressure fluid move
What is common cause of stomach ulcer?
Helicobacter pylori- damage protective mucous layer
H. pylori multiply in the mucus layer of the stomach lining
Secrete enzyme called urease converts urea to ammonia
Ammonia protects bacteria from stomach acid
Function mucous cell?
Produce HC03-
Protective lining and buffer against HCl
Outline acid production? (equation)
CO2 + H2O ↔ H2CO3 ↔ HCO3- + H+
Function CCK?
Stimulate gallbladder contract and release stored bile into intestine
Stimulate secretion pancreatic juice
What releases CCK?
I cells- upper SI in response fat and protein
