What is GORD
A
Gastro-oesophageal reflux disease
Stomach refluxes through the lower oesophageal sphincter and irritates lining of oesophagus

GORD presentation
A
Dyspepsia (indigestion) Heartburn Acid regurgitation Retrosternal or epigastric pain Bloating Nocturnal cough Hoarse voice

GORD investigations
A
Endoscopy if any concerning features e.g. weight loss, vomiting blood or coffee grounds, meleana, dysphagia, not responding to treatment,
low Hb, raised platelets
Test for Helicobacter pylori

GORD lifestyle advice
A
Reduce tea, coffee and alcohol Avoid smoking Avoid acidic foods/trigger foods e.g. tomatoes, spicy foods... Weight loss Smaller, lighter meals Stay upright after meals

GORD management
A
Acid neutralising meds (antacids) e.g. Gaviscon, Rennie
PPI’s e.g. Omeprazole 20mg 30mins before breakfast
H2 receptor antagonist e.g. ranitidine
Surgical laparoscopic fundoplication - tying fundus of stomach around lower oesophagus to narrow LOS
