If goodpasture’s syndrome has auto-antibodies against glomerular basement membrane (Anti-GBM disease), why does the person get haemoptysis?
The auto-antibodies are for type 4 collagen which is found in the glomerular basement membrane AND alveolar capillary BM
What is the treatment for goofpasture’s disease?
Plasmapheresis + immunosuppression
What auto-antibody is found in Wegener’s granulomatosis and what is it specific for?
c-ANCA > proteinase 3
How does methotrexate work?
inhibits metabolism of folic acid (folic acid reductase) > preventing DNA synthesis + cell proliferation
What tests should you carry out to Ix Wegener’s?
CXR, Urinalysis, renal biopsy, CT chest, serology (for auto-antibodies)
What is the treatment plan for Wegener’s granulomatosis?
- Prednisolone + cyclophosphamide
- rituximab
- Maintain: methotrexate + azathioprine
How would you treat severe Wegener’s?
Plasmapheresis.
What prophylactic treatment should you give? Why?
Co-trimoxazole to prevent pneumocystis jirovecii infection
