Accident Notification Procedures: When must the NTSB be notified?
Accident as defined by NTSB 840. Flight control system malfunction or failure. Inability of any required flight crew member to perform normal duties as result of injury or illness. Failure of any internal turbine engine component that results in the escape of debris other than out the exhaust path. In-flight fire. Aircraft collision in flight. Damage to property other that the aircraft estimated to exceed $25,000. Aircraft overdue (45 min) and is believed to have been involved in an accident.
Accident Notification Procedures: When must a crew member File a report
When he/she deviates from a FAR, sent to Honolulu FSDO and DofO not later than 10 business days after the deviation
Maintenance Procedures: Pre Flight duties, Pilot checks
Annual inspection (H369E), 100hr inspection, Pitot/Static system (altimeter/xpndr), uncorrected discrepancies including preceding 3 days

record any mechanical irregularities and report them to the DOM

Fueling Procedures:
No smoking, pax or other personnel not directly engaged with fueling within 50 feet. Only trained and qualified personnel will refuel. Pilot at the controls. Available fire extinguishers. Not within the vicinity of a thunderstorm
Fueling procedures: Fuel contamination
if contaminated fuel is discovered during preflight, the PIC will continue drawing fuel samples until the samples are no longer contaminated. A PIC who suspects or determines that the A/C fuel system is contaminated (a clean sample can not be obtained) the PIC will notify the DOM and will not fly the aircraft until resolution of the matter
