MOA of H2 antagonists
-They inhibit gastric acid secretion by blocking the effect of histamine at the histamine H2-receptors in the parietal cells of the stomach
examples of H2 antogonists
Cimetidine (Tagamet),
Ranitidine (Zantac),
Nazitidine (antizid)
Cimetidine drug interactions - MOA
inhibit metabolism
CImetidine drug interactions - drugs
Diazepam, Warfarin, Theophilline, Phenytoin
Cimetidine possible side effects
Male lower spermcount, loss of libido, impotence, gynaecomastia
Ranitidine, Nazitidine s/effects, d/i comparison to Cimetidine
-They do not have an effect on male sexual function & sperm
count
-Do not cause gynaecomastia
-Unlike cimetidine, have little or no potential for drug
interaction
Proton Pump inhibitors MOA and examples
-They reduce gastric acid secretion by blocking the absorption of
hydrogen ions (protons) into the parietal cell by inhibiting H+/K+-
ATPase enzyme system (proton pump)
Omeprazole indications
Uses: Zollinger-Ellison syndrome: rare, gastric producing tumor (Drug of choice)
- Peptic ulcer - Reflux oesophagitis - Treatment & prevention of NSAID-induced erosion - Combined with certain antibiotics to eradicate Helicobacter
Omeprazole possible side effects
Adverse Effects: Headache, Diarrhoea, Rashes, Dizziness
- Nausea, Vomiting, Pruritus, Alopecia - Gynaecomastia, Blood disorders, Constipation
Omeprazole Drug interactions
- Diazepam Omeprazole inhibits the metabolism
- Warfarin & enhances the effects of these
- Phenytoin etc drugs
Lansoprazole difference to Omeprazole in indication
Not for Zollinger-Ellison syndrome
