Vibro cholera, Enterotoxigenic E. coli, Enteropathogenic E. coli, rotavirus, norovirus
Inflammatory diarrhea pathogens
Salmonella, Shigella, Campylobacter, Yersinia, Enterohemorrhagic E.coli, Enteroinvasive E. coli, Enteroaggretative E. coli, C diff
treatment
- Fluid Replacement
- anti-diarrheals (Watery only)
- Probiotics
- Fecal transplant
C diff initial infection mild or moderate clinical data
WBC < 1.5 * baseline
C diff initial infection mild or moderate drug
metronidazole 500 mg PO q8h x 10-14 days
C diff initial infection severe data
WBC >15,000
Scr > 1.5 x baseline
C diff initial infection severe drug
vancomycin 125 mg PO Q6h x 10-14 days
C diff initial infection severe and complicated data
hypotension, shock, ileus, toxic megacolon
C diff initial infection severe and complicated drug
vancomycin 500 mg PO q6h + motronidazole 500 mg PO q8h, may add PR vanc for complete ileus
C diff first relapse drug
treat with same agent as before, but if severe treat as severe
C diff 2nd relapse drug
vancomycin (with taper and/or pulse dose)
C diff alternate drugs
Rifamixin, Nitrazoxanide, IVIG, Fidaxomicin, Tigecycline, fecal transplant, surgery
Travelers diarrhea treatment
- ORS
- antimotility agents
- 500 mg of cipro BID or 400 mg norfloxacilin BID for 3-5 days
Travelers diarrhea alternate drugs
azithromicin, rifamixin
