Tests only for presence of toxin B gene, so can be positive for carriers. Proposed better test: EIA for GDH followed by EIA for toxin B, with or without gene PCR

A

Fulminant C.diff is severe CDI with hypotension, shock, ileus, megacolin

Severe treated with oral vanc 125mg QID, fulminant treated with oral vanc 500 QID, IV flagyl 500mg q8h. Consider vanc enemas in those with ileus.
