GI Flashcards
What is the treatment for oesophageal vatical rupture?
Rescue TIPS
Endoscopic banding
Prophylactic antibiotics (as patients are risk of infection)
Vasopressin (Terlipressin) to manage low blood pressure
What are the three classes of drugs used in BOTH UC and Crohn’s disease?
Corticosteroids
Thiopurines
Biologics
Which drugs are used for UC alone?
5ASA’s (aminosalycates)
Which drug is used for Crohn’s disease alone?
Methotrexate
immune modulating diets are also used to treat Crohn’s disease alone
Give 3 examples of steroids used for UC and Crohn’s?
IV hydrocortisone
methylprednisolone
prednisolone
What should be given with prednisolone to help prevent bone side effects of steroids?
calcium and vitamin D supplementation
What are three side effects of steroids?
Immunosuppression Impaired glucose tolerance (borderline diabetic becomes diabetic) Osteoporosis Weight gain Cushingoid appearances
Give an example of a 5ASA?
Sulphasalzine
Mesalazine
Pentasa
What is the aim of 5ASA’s?
Induction of remission in mild to moderate ulcerative colitis
Maintenance of remission in UC
Reduce risk of colorectal cancer as they aim to reduce the inflammation
Give an example of thiopurines?
Azathioprine and mercaptopurine
steroid sparing drugs - this means less dose of steroid is required when these drugs are also used
What is the aim of thiopurines?
Maintenance therapy for UC and Crohn’s
What does azathioprine do?
Essentially prevents T cell clonal expansion in response to antigenic stimuli
Allows T cell apoptosis
What does TPMT do?
TPMT: Thiopurine methyl transferase metabolises azathioprine to its active agent.
