A
- How many pregnancies
- Were they until full term
- Mode of delivery (doesn’t matter if they were induced)
- Weight of babies

What to ask about in current obstetric history
A
- Gestation from dating USS (to find out estimated DOD)
- Gravidity & parity
- Any problems
- Any tests done, including scans & why
- Antepartum haemorrhage
- Pre-eclampsia
- Abdo pain (common after 24 weeks)
- PPROM

A
- Year of birth
- Antenatal problems
- Gestation at delivery
- Induction of labour
- Mode of delivery
- Gender (+name)
- Birth weight + centile
- Postnatal problems
- Pregnancy losses: miscarriage, termination, ectopics

What details to ask about pregnancy losses
A
o At how many weeks
o How it was managed (conservative, tablets, theatre)

Other details to always ask a pregnant woman
A
- Foetal movements >20 weeks
- Any tightening, contractions, abdominal pain
- Any fluid or blood from vagina
- Urinary complaints
- Bowel complaints
