how do you test for celiacs?
A
- screening test: tTG-IgA (tissue transglutaminase Ab) with pt on gluten diet
- Definitive diagnosis by small bowel biopsy or by note of improvement on gluten free diet

what is rome criteria for IBS?
A
2 of 3:
- abdominal pain relieved with defecation
- abdominal pain associated with increase or decrease in still frequency
- abdominal pain associated with harder or looser stools

management for IBS
A
- fiber modification, macrobiotic diet

types of infectious diarrhea
A
- food poisoning: 2-6 hrs after ingestion (salmonella, norovirus, campylobacter, e. coli, listeria) -> tx supportive
- traveler’s diarrhea: 24-72 hr incubation period. e. coli or viral. tx supportive– abx possible - cipro 500 mg BID x 5 days. Pepto-Bismol can help.
- giardiasis: incubation 10-20 days. giardia lamblia. tx metronidazole 250 mg tid x 7 days
- bloody diarrhea: campylobacteri jejuni, enterohemorrhagic e.coli, Shigella toxin E. coli (STEC), shigella, c. diff

A
- stool culture
- c.diff assay
- protozoal testing

treatment of c. diff
A
- stop inciting abx
- metronidazole or vancomycin (PO) - 10-14 days
