Head
Pain or discomfort, dizziness/lightheadedness, presyncope, tinnitus, facial droop, slurred speech
Neck
Pain or discomfort, difficulties swallowing, suprasternal reactions, trachea position, presence of JVD, stridor
Chest
Pain or discomfort, shortness of breath, accessory muscle use, lung sounds (auscultation), medical patches (must be removed), pacemaker scars
Abdomen
Pain or discomfort, needle marks, nausea/vomiting, pulsating mass (do not palpate)
Pelvis
Pain or discomfort, last bowel movement, urinary pattern questions (polyuria, hematuria, dysuria, foul smelling)
Extremities
Pain or discomfort, numbness or tingling, edema, CMS evaluation, presence of needle marks, wounds
Back
Pain or discomfort, medical patches (must remove), kyphosis/lordosis, scoliosis
What are the standards of assessment?
Preformed on everyone, no matter the complaint; lung sounds (auscultation), medical patches on the chest must be removed, needle marks in the abdomen, CMS evaluation, medical patches on the back must be removed
