a patient come in due to SOB
they have myotonic dystrophy

what can be the cause of the SOB

A

patient has reduced left heart function - reduced ejection fraction due to his heart muscle deteriorating

he also gaimed pneumonina and then also a PE (was sob for 5 weeks then last week got worse- due to the pneumonia and the two PE exaserbatig the SOB)

give furusomide to drain off the fluid round his lungs due to congestion in lungs cus heart failure. give apixaban for the PE. but then this increases fluid for kindeys to deal with and also the fursomide is nephrotoxic the kidneys dont work as well and get heamaturia and bladder distention becasue constipated and blocked.

catheter in for help distended bladder and laxatives.

speak to conusltant - superior- to decide what to do about the bleed injurung the kidneys but then also need anticoag to stop dying from the PE

the decision for this case was stay on apaixaban as need dissolve the clots as the main priority

what is dapagliflozin

A

treat t2 diabates- if cant have metformin

treat symotomatic heart failure with reduced ejection fraction - good if the pt has renal failure

CKD

works by: Reversibly inhibits sodium-glucose co-transporter 2 (SGLT2) in the renal proximal convoluted tubule to reduce glucose reabsorption and increase urinary glucose excretion

SE: can caues DKA!
