Employee engagement.

And this is the employee engagement form just indicating a few things that we already went over about the importance of 100% participation.
First employer tax savings but I seen every employee in insurance we have the best participation, the greater participation by the employees the greater tax savings for the business.
Second the employees Goodwill. It is important that all employees are equally offered this benefit program and receive the no-cost policies for the goodwill of the employees. You know your employees are going to pay for these introductory offers.
And last is staying compliant with section 125 so we are going to take that burden from you and keep the documentation we needed to make sure everyone has been equally offered this program so we will need to make sure we see every employee.
If you will just initial all three indicating you understand how our program works.

Group term

So ___, This right here will give us permission to offer our group term, because it is only offered exclusively as an employee benefit. It says here that you are not buying or endorsing it, but rather allowing us to offer it as a group. I just need you to authorize here.

Pedro deductions

A

This is very short so you’re welcome to read it but basically it says that you’re willing to do payroll deductions for you employees if they enroll and if you look here at paragraph four, this is the most important part to protect you. It says if an employee leaves abruptly or is terminated you are NOT responsible for unpaid premiums. I just need you to authorize it here.

Roster

Great! One more thing before I go I just need a quick employee listing that shows their names and our dates so we can determine who is eligible, and this also helps us with the compliance I discussed with you a few moments ago. Now is that something you can get real quick or will (payroll contact) get that for us?
Great! I’d love to her/him before I go since she’s the person that will be helping us service your employees. (meet with foreman or the production manager also if possible)

The process of filling out paperwork should always be conversational. Even in asking for the roster and location and time of enrollment. Be sure to me your point of contact for the day of enrollment before you leave.
Do you not leave with out the roster!
