Gastric Cancer Flashcards
what are the types of gastric cancer?
adenocarcinoma, lymphoma, carcinoid tumour (G cells in stomach) , leiomyosarcoma (smooth muscle cells)
layers of mucosa?
- epithelial
- lamina propria
- muscularis mucosa
foveolar cells secrete?
mucus
parietal cells can be found where and secrete what?
body and fundus, HCL
chief cells secrete?
pepsinogen
g cells secrete?
gastrin- stimulate parietal cells
most common type of gastric cancer?
adenocarcinoma
types of adenocarcinoma?
intestinal- well differentiated, diffuse
what causes intestinal cancer?
Hpylori cagA causing gastritis
where can you find intestinal cancer?
lesser curvature of antrum, large irregular ulcer with heaped edges well differentiated
diffuse type of adenocarcinoma?
mutations of CDH1 - E-cadherin (epithelial cells stick together and controls cell cycle) increased ability to spread
what can diffuse type cause?
gastric linitis (leather bottle)invading submucosa, hisyologically signet ring cells in connective tissue
what can diffuse type cause?
gastric linitis (leather bottle)invading submucosa, hisyologically signet ring cells in connective tissue
lymphomas arise because?
chronic infection of H pylori causes b cell proliferation resulting in mutation.can show diffuse lymphocytes in MALT
carcinoid tumours?
neuroendocrine cells g from mucosa as polyp, it is well differentiated, also found in pancreas and intestine
leiomyosarcoma?
smooth muscle cells- extremely rare
look like spindle, epithelial, or undifferentiated cells
complications of adenocarcinoma?
metastatsis: peritoneum, lymph nodes umbilicus and virchows node, liver and ovaries (krukenberg tumour)
paraneoplastic syndrome
complications of adenocarcinoma?
metastatsis: peritoneum, lymph nodes umbilicus and virchows node, liver and ovaries (krukenberg tumour)
paraneoplastic syndrome
paraneoplastic syndrome?
- leser-trelat sign ( seborrhoeic keratosis) cancer- growth factors- keratinocytes.
- polyarteritis nodosa- inflamamtion/necrosis of medium arteries. MI/kidney failure
- trousseau syndrome migratory thrombosis cancer cells- tissue factor- coagulation cascade leads to thrombosis
- pseudoachalasia syndrome
risk factors of gastric cancer?
age, fh, smoking alcohol obesity
risk factors of intestinal type?
hpylori, male, type a blood, diet rich in nitrates, nitrosamines and salted food. autoimmune gastritis, pernicious anaemia, achlorhydria
symptoms?
malaise, weight loss, dyspepsia, loss of appetite, epigastric pain, nausea and vomiting, anaemia, haematemesis, melena
signs of gastric cancer?
acanthosis nigricans, sister mary joseph syndrome (enlarged belly button lymph nodes), troisiers sign enlarged virchows node
how is diagnosis made?
endoscopy
