Biliary colic
Enzyme produced by the duodenum that stimulates the gallbladder to contract after eating (especially. fatty food)

Acute cholecystitis
Main 2 symptoms of Acute Cholecystis?
Pain (biliary colic)
Fever
Other symptoms of Acute Cholecystitis? (2)
Nausea/Vomiting Back pain (radiation)
What is Murphy’s sign? What does it show?
- Pain on deep inspiraition when the RUQ is palpated

Investigations for Acute Cholecystitis?
- Bloods: FBC, LFTs, CRP
2. Abdominal USS
What can you do if the Abdominal USS is inconclusive but there is still a high suspicion of gallstones?
MRCP
What happens to each of these markers in Acute Cholecystitis?
- WBC
- CRP
- AST/ALT
- ALP, GGT, bilirubin
WBC = raised
CRP = raised
AST/ALT = normal
ALP, GGT, bilirubin = normal (1/3rd can have raised markers - stone in CBD)
Risk factors for gallstones (5)
Fat Female Fertile Forty (young women) Fair (Caucasian)
EXTRA: FOOD - Family history, OCP, Old age, Diabetes
Management of Acute Cholecystisis? (6)
- IV fluids
- Abx
- Analgesia
- Antiemetics
- Laparoscopic Cholecystectomy
- +/- ECRP clearance of CBD
What would the abdominal USS show in Acute Cholecystitis? (3)
- Thickened gallbladder walls
- Gallstones or sludge in gallbladder
- Duct dilatation
What is gallbladder sludge?
Early signs of gallstones formation.
What are the 3 main types of gallstones?
- cholesterol stones (most common - 80% of cases)
- (Bile) Pigment stones
- Mixed cholesterol/pigments
Complications that can arise from Acute Cholecystitis?
- Mirizzi Syndrome
- Chronic cholecystitis
- Gallbladder empyema (pus)
