What is the most common cause of obstructed flow within the gallbladder?
Gallstones
Risk Factors
-age (middle age)
-family history
-obesity
-rapid weight loss
-hyperlipidemia
-Native American or hispanic
-female
Gallstones form when what interacts?
-abnormal bile composition
-biliary stasis
-inflammation of gallbladder
S/S
-RUQ pain (radiates to right shoulder)
-fever with chills
-tachycardia
-n/v
-anorexia
Gallbladder Disease is also known as
Cholecystitis
Acute Cholecystitis is what
-follows obstruction of cystic duct
- increased pressure in GB
Chronic Cholecystitis is what
-repeated bouts of acute
-often asymptomatic
Diagnostic:
-serum bilirubin
-cbc/wbc
-serum amylase/lipase
-abd. x-ray
-ultrasound of GB
-ERCP (endoscopy retrograde cholangiopancreatography)
What is ERCP
-ERCP (endoscopy retrograde cholangiopancreatography): through throat to assess narrowing, can reomve stones and place stent
Treatment options
-Laproscopic Cholecystectomy
-Cholecystectomy
-Lithrotripsy
Nursing interventions:
-Assist w/ ambulation
-prevent pneumonia: deep breathing and cough
-prevent infection: only baths
-report red and swelling at incision site
-monitor vital q4
-incentive spirometer q1-2 hours
Diet
decrease fat, decrease cholesterol, decrease calories, increase protein
-admin fat soluble vitamins (A,D,E,K)
Meds:
Ursodiol and Chenodiol
Med therapeutic effects:
can take up to 2 years or more
