A
Jaundice and encephalopathy in a patient with a previously normal liver.

what are the common causes ?
A
Paracetamol Fulminant viral Drugs HBV Non A-E hepatitis

A
AFLP Mushrooms Malignancy Wilson’s Budd chiari HAV

what are the signs of FHF?
A
- Encephalopathy
- Hypoglycaemia
- Coagulopathy- This is a defining feature of FHF
- Circulatory failure
- Renal Failure - creatinine starts rising
- Infection

what do LFTs show?
A
hyperbilirubinaemia, elevated liver enzymes

what do PTT show?
A
elevated - good marker for FHF

what does metabolic panel show?
A
elevated urea and creatinine, weird K, PO4, and MG

what maya FBC show?
A
leukocytosis, anaemia

what maya ABG show?
A
metabolic acidosis

what other tests would you do?
A
- serologies
- ceruloplasmin
- CXR - possible aspiration pneumonia
- USS

what is the treatment?
A
supportive, inotropes and fluids, renal replacement management of ICP transplant
