TBI
Aim for BP >120
NS 40ml/kg
If BP <100 after 40ml/kg, consult for more. If no consult, give a further 20ml/kg
Anaphylaxis
NS up to 40ml/kg
consult for more, if no consult available, a further 20ml/kg
Burns
If >15yrs with TBSA >15%
TBSA% x pt weight over 2 hours
If 12-15 yrs and TBSA >10%
3 x TBSA% x pt weight over 24 hrs, half in the first 8
Haemorrhagic Hypovolaemia
If BP <70
NS 250 ml as required (max 2L)
consult for further doses, consider blood products
Also applies for ruptured AAA, GI bleeds, pregnant trauma pts.
Inadequate perfusion (sepsis, dehydration)
NS 20ml/kg over 30 mins if chest clear and MICA unavailable
DKA
NS 20ml/kg titrated to perfusion
Consult for more
Consider reduced volumes for elderly or impaired renal/cardiac function
APH
if < adequate perfusion:
NS 40ml/kg titrated
Consult for more, if no consult available:
Further 20ml/kg
PPH
If BP <90:
NS 40ml/kg titrated
