what is a surgical flap
A
seperation of tissue from surrounding tissues except for its base
in perio: we use flap to remove soft tissue and reach periodontal structures

full thickness (mucoperiostal)
- epithelium
- connective tissue
- periosteoum
- most commen when bone resection or regeneration
partial thickness (split thickness-mucosal)
- periosteoum not included
- in mucogingival surgeries to leave blood supply
*a combination might b used 4 crown lenght n some mucogingival procedures

A
- accesibility 4 root planing
- establish the morphology of area easy 2 clean n with less plaque retention
- correction of gingival overgrowth
- shifting gingival margin apically for plaque retaining restorations
- pocket depth reduction! (apically repos)
- thickening of marginal bone

flap contraind
A
poor patient coop poor plaque control high caries rate uncontrolled medical conditions -diabetes -hypertension -blood disorders -unstable angina -myocard inf within 6 m

advantages over curetage or gingivectomy
A
direct vision 4 optimum s & r p little periodontal tissue loss pocket epithelium entirely removed no defects on bones coverd by flaps no open wounds persist post op
disad: when flaps r repositioned apically: hypersensitivity n poor aesthetic
