Whats the difference between a pseudocyst and a regular cyst?
pseudocysts lack an epithelial lining, the capsule in a pancreatic pseudocyst is fibrosis from inflammation of surrounding tissue
what is the puestow procedure? when indicated?
Longitudinal pancreaticojejunostomy: patients w refractory chronic pancreatitis and dilated main pancreatic duct >8mm
what is a frey procedure?
resecting the head of the pancreas and drainage via pancreaticojejunostomy, used in patients w ductal dilatation localized to the head and body of the pancreas
what is the berger procedure?
resection of the pancreatic head with preservation of the duodenum, a sleeve of panc head tissue remain behind (uncommonly performed)
describe technique of how you open pancreatic duct in puestow procedure?
open duct anteriorly along length, medially to the GDA and laterally into the tail beyond all pancreatic duct strictures
What’s ransons criteria?
Presentation: age >55 WBC >16 Glucose >200 AST > 250 LDH >350 After 48hrs: Hct decreased by 10%, BUN increased of 5, Ca <8, PaO2 <60, base deficit >4, fluid sequestration >6L
most important risk factor for necrotizing pancreatitis?
obesity
ARDS in setting of pancreatitis is related to what?
release of phospholipases
coagulopathy in setting of pancreatitis is related to what?
release of proteases
management of pancreatic cysts not associated with pancreatitis?
resection! may be IPMN or mucinous cystadenocarcinoma
MCC splenic vein thrombosis?
chronic pancreatitis
tx of isolated bleeding gastric varices?
splenectomy (most likely 2/2 splenic vein thrombosis)
