2. Lifting gears and WAH unsafe practices
3. LOTO & open pipe not cap need improvement
4. Cold venting safe practice need improvement
5. Liquid level reading mismatch.
6. SOL/NOL setting in HMI need improvements
7. Shift handover inconsistent fill with daily prod report, and not daily review by supervisor.
8. IMT room facility and adequacy.
9. DCS room mixed with Electrical room.
10. combustible material persist in electrical panel room.
11. temperature in DCS and electrical room not optimal, supposed to set at 25.5 deg C.
