Patient Safety
the most fundamental principle of any health care service
First, do no harm
Defined as an unintended act(either omission or commission) or
One that does not achieve its intended outcome, the failure of a planned action to be completed as intended ( an error of execution) or
The use of wrong plan to achieve an aim (an error of planning)
A deviation from the process of care which may or may not cause harm to the patient
Medical Error
This type of medical error occurs when injury is caused by medical management rather than the underlying condition of the patients
Adverse event
This medical error prolongs hospitalization, produces disability at discharge or both
Classified as preventable or unpreventable
It is a care that falls below a recognized standard of care
Negligence
A care reasonable physician of similar knowledge, training and experience would use in similar circumstances
Standard of care
An error that foes not result in patient harm; provides the opportunity to identify and remedy system failures before the occurence of harm
Near miss
A unexpected occurence involving death or serious or psychological injury
Sentinel event
This theory recognizes that there are certain high-risk industries and organizations that have achieved very low accident and error rates compared to what would be expected given the inherent risks involved in their daily operations

Leaders should be open to challenging long-held beliefs and continuosly look at data, benchmarks and performance metrics to prevent simplification
This principle states that HRO actively communicate with employeees of all levels and show respect for individuals by taking their concerns seriously and listening to ther feedback

HROs understand that getting a good understanding of the current situatin involves working with frontline employees because they are closer to the work
