Q

FAB indication of acute leukemia

A

> 30% blasts in BM & >5 % in blood

M0- acute myeloblastic leukemia (AML)

M3- Aute promeylocytic leukemia ( APL)- hypergranular promyelocytic

M4- acute myelomonocytic leukemia (AMML) - naegeli

M5- acute monocytic leukemia-a&b ( AMoL)- schillingg (a)

M6- erytholeukemia (AEL) - Di Guglielmo syndrome

m7 - megakaryoblastic leukemia ( AMegL)

Q

ALL

A

acute lymphoblastic leukemia

common in children

increased lymphoblasts

L1 - scanty cytoplasm, small
L2 large, lots of cytoplasm, indented nucleus
L3 (Burkitts) - large lots of basophilic cytoplasm, vacuoles

splenomegaly

NRBCs

thrombocytopenia

> 50% lymphoblasts

serum uric acid & LD may be increased

Q

AMoL

A

M5

bleed disorder
splenomegaly
infections
extramedullary : lymoh nodes, skin, liver, spleen, CNS

M5a - schillings type
poorly differentiated
monoblasts
chromosome 11 band q 23
younger age
M5b
well differentiated
promonocytes
nucleus has more folding/ lobulation
adults

Q

AMML

A

M4- naegleri

monocytic & granulocytic differentiation
the myeloblast stains positive for MPO or SBB

M4Eo
-AMML associated with BM eosinophilia
- inversion of chromosome 16
- shows myeloid and monocytic differentiation
stains postive for MPO, SBB & NSE

Q

APL

A

Acute promyelocytic leukemia - M3

> 30% of non RBCs

heavy granulation ( sometime obscures nucleus )

abundant cytoplasm

bundles of auer rods ( faggot cells)

translocation of 15; 17 that leads to gene fusion

bleeding episodes
- DIC ( abnormal promyelocytes produce tissue like substances that triggers DIC)

Q

what is the lap score of a leukmoid reaction vs CML

A

leukmoid reaction has an increased lap score

CML has a low lap score unless there is an infection ( also left shift & higher WBC count ) `
