Cardiac Assessment
A
- Auscultate and palpate carotids
- JVD
- PMI
- Heart sounds APTM
- Angle of Louis (I & P)
- Assess peripheral circulation (I & P cap refill)
- inspect fingers for clubbing, cyanosis, joints, skin and determine dominant hand
- assess radial, bracheal, posterior tibialis, dorsalis pedis bulses and grade
- Inspect bilat LE for vericosities, edema, clubbing, cyanosis (report and grade)

A
- Inspect thorax, symmetry with resp (F & B)
- Percuss lung fields
- tactile fremitus (“99”)
- auscultate lungs (8 points)
- egophony (auscultate “e” at 8 points) (hearing “a” = consolidation)

A
- Inspect (supine)
- bowel sounds (B & D)
- Auscultate for bruit
- Percuss abdomen x4Q
- Palpate x4Q (light then deep) (on deep identify organs, liver, gallbladder, spleen, bladder)
- liver edge and span, dullness to percussion, measure (7 cm)
- kidneys with ballottement, (deep breath, no tenderness or masses)
- percuss CV angle (no pain) pain = kidney infection

Spine assessment
A
- palpate spinous processes for allignment and tenderness
- palpate paraspinal muscular area and identify parts of the spine (cervical, thoracic, lumbar, sacrum, coccyx)
- ROM of spine (flexion, extention, lateral bend) in thoracic and lumbar spine
- identify C7 / T1 and check cervical ROM (head up down and lateral)
- test proprioception - Rhomberg and pronator drift bilaterally

Muscuoskeletal
A
- ROM active and passive:
* fingers, wrists, elbows, shoulders
* hips, knees, ankles, feet, toes - strength and grade
* UE (squeeze hands, push, pull = strength is a 5
* LE (Legs out &, feet up against resistance), strength is a 5 - coordination and motor skills-
* finger to nose, heel to shin
* rapid alternating movements bilaterally - sensory ability- dull/sharp in all 4 extremities
- deep tendon reflexes-
* biceps, triceps, brachioradialis, triceps, patellar, and achilles bilaterally - vibratory sense of LE
- position sense of great toe bilaterally
- plantar reflex
● Toes curled down, plantar reflex intact, Babinski negative bilaterally
