What is the main feature of fibromyalgia?

A

Chronic widespread pain. To be chronic it must be >3 months in duration. To be widespread it must be found on both sides of the body, above and below the waist and along the axial spine.

  • Difficulty sleeping
  • Poor concentration/memory
  • Extreme + constant tiredness that doesn’t improve with sleep or rest

What are the signs of fibromyalgia?

A

There are 18 well recognised ‘tender points’ that must be palpated - 4 anterior and 5 posterior. Patients reporting pain in 11 or more of these sites > fibromyalgia. Pressure should be applied with the pulp of the thumb until the examiners nail blanches.

How do you diagnose fibromyalgia?

A
  1. Widespread pain index (WPI) and symptom severity score (SSS) - WPI >/= 7 + SSS >/=5 OR WPI 4-6 + SSS >/=9
  2. Generalised pain: 4/5 regions
  3. Symptoms present >/= 3 months
    Do not need to rule out other conditions if criteria 1-3 are met.

A
  • Exercise (strong evidence of benefit)
  • CBT
  • Acupuncture
  • Hydrotherapy
  • Meditative movement
  • Screen for anxiety and depression and treat appropriately

  • Duloxetine

A

Severe pain or increased disability at presentation, previous significant pain episodes, multiple site pain, non-organic signs, iatrogenic factors.
