Q

fibroids - management (if needed)

A

1° - IUS ± NSAIDs
2° - tranexamic acid, COCP

short term GnRH agonist - shrink it
UAE - devascularise + involute it under LA
endometrial ablation
myomectomy
hysterectomy

16
Q

what is hyaline degeneration of fibroid?

A

fibroid gradually outgrows blood supply, may → to central necrosis → cystic degeneration: cystic spaces at centre (?)

17
Q

how can fibroids cause menorrhagia? what type of fibroid, management?

A

submucous fibroid distorts endometrial cavity
remove submucous component (daycase; may try other measures before eg IUS, tranexamic acid; can shrink with GnRH pre-surgery)
