Fentanyl - indications
A
- Sedation to facilitate and maintain intubation
- Sedation for transthoracic pacing
- Sedation for synchronised cardioversion
- CPR interfering Pt
- Analgesia

Fentanyl - reasons to prioritise fentanyl over morphine
A
- Hypersensitivity or allergy to morphine
- Known renal impairment/failure
- Short duration of action desirable
- Hypotension
- Nausea & vomiting
- Severe headache

Fentanyl - contraindications
A
- Late second stage labour
- Known hypersensitivity
- Serotonin syndrome
- Monoamine oxidase inhibitors within the past 14 days

A
- Elderly/frail
- Impaired hepatic function
- Respiratory depression
- Current asthma
- Patients on monoamine oxidase inhibitors
- Known addiction to opioids
- Rhinitis, rhinorrhea or facial trauma (IN)

- Respiratory depression
- Apnoea
- Rigidity of diaphragm and intercostal muscles
- Bradycardia

Fentanyl - why are monoamine oxidase inhibitors a contraindication?
A
MAOI’s are usually used to treat depression and increase the amount of serotonin within the body - large amounts of serotonin can cause serotonin toxicity (both fentanyl & morphine also increase levels of serotonin resulting in increased risk of serotonin syndrome)
