Endometriosis overview
cyclical pain
dysmenorrhoea, dyspareunia, chronic pelvic pain
try COCP, GnRH then laparoscopy
Functional ovarian cysts
causes acute pain if it twists or bleeds
reduced fertility
Rx - remove ovary, untwist, laparotomy/laparoscopy
Adnexal torsion
torsion
haemorrhage
cyst rupture
Pregnancy
Ectopic rupture
normal
miscarriage (with bleeding)
Pelvic inflammatory disease - presentation
lower abdominal pain (uni or bilateral) constant/intermittent pain deep dyspareunia discharge intermenstrual/post coital bleeding dysmenorrhoea fever
causes of PID
STIs
iatrogenic (laparoscopy, IUD, ToP)
CHLAMYDIA
PID investigations
STI swabs
CRP, cultures etc if unwell
TVS if abscess
if still uncertain - laparoscopy
PID treatment
ceftriaxone
doxycyline
metronidazole
investigation of acute pelvic pain
urinalysis high vaginal swab pregnancy test FBC TVS laparoscopy
investigation of chronic pelvic pain
infection screen CA125 urinalysis TVS laparoscopy
