name, strength per unit, and amount. Also the date, and must be destroyed in a way compliant with law, must be witnessed by a practitioner or pharmacist and both must print and sign names. Exception- open ampules remains may be destroyed without a witness
when must you report loss of theft for targetted substances
within 10 days for discovery
if you buy/receive a targetted substance from another pharmacy, what must be recorded?
name+brand (if applicable), strength per unit, # of units, name and address of licensed dealer/rxist/hospital it came from, date

what is a verbal rx narcotic
contains 2 additional medicinal ingredients in therapeutic doses and is not intended for parenteral use and doesn’t have; diacetylmorphine, hydrocodone, methadone, oxycodone, pentazocine
narcotics must be (in this form)
written or faxed only
compounds or formulations containing these five products will always be narcotics
