Allergic Bronchopulmonary Aspergillosis
Results from an allergy to aspergillus spores
Fx:
- wheeze, cough, dyspnoea
- proximal bronchiectasis
- Bronchocoeles (finger in glove) on CT
Histoplasmosis
- Fever
- Malaise and weight loss
- Dry cough
- URTI sx
- Retrosternal lain
O/E
- Hepatosplenomegaly
- Erythematous cutaneous lesions
Churg-Strauss Syndrome
A small vessel vasculitis with eosinophilic infiltration and granulomatous formation
Fx:
- chronic sinusitis
- asthma
- eosinophilia
- malaise, myalgia, fever, weight loss
Lung -> asthma, pneumonitis Skin -> purpura, SC nodules Heart -> HF, myocarditis GI - > gastroenteritis, ischaemia, bleeding Renal -> GN, HTN CNS -> mononeuritis multiplex

What was Idiopathic Pulmonary Fibrosis (IPF) previously called?
Cryptogenic fibrosing alveolitis
Define IPF
Chronic lung condition characterised by progressive fibrosis of lung interstitium.
50-70yrs, M>F
IPF Fx:
- progressive exertions dyspnoea
- bibasal creps
- dry cough
- finger clubbing
EGPA fx:
- Atopy (asthma, rhinitis, sinusitis)
- Eosinophilia > 10% (+ pulm. Infiltrates)
- Necrotising multi-system small vessel vasculitis
- Mononeuritis multiplex
- pANCA +ve
Eosinophilic pneumonia
- subacute resp sx
- alveolar/ blood eosinophilia
- CXR: peripheral pulmonary infiltrates

- PTH-rp -> hypercalcaemia
