What does the USPSTF say about clinical breast exams?
Not enough evidence to assess the risks/benefits beyond the mammo starting at age 50
What does the ACS say about clincal breast exams?
Should be done every 3 years for women in their 20s and 30s and every year for women over 40
Cervical cancer screening guidelines
Pap every 3 years for women 21-29; Pap every 3 or Pap with HPV every 5 years for women 30-65
Who are the women at higher risk for cervical cancer who need to be screened more freq than the recommended schedule?
Immunosupp, HIV pos, Hx of CIN2 or CIN3 or cancer, exposure to DES in utero
When can you stop screening?
At age 65, if the woman has had a normal Pap in the last 10 years
What is the effect of smoking on cervical cancer? Breast cancer? Endometrial?
increases cervical cancer, no impact on breast cancer, decreases endometrial
What is the Bethesda system for reporting the cervical cytology for Pap smears?
First, they talk abou the adequacy of the sample (must have over 5000 squamous cells with sufficient endocervical cells); Second, they categorize the results into CIN or malignancy; Third, they interpret the results into ASCUS, LSIL, HSIL, or squam cell carcinoma
What does Gardasil cover?
HPV strains 6,11 (warts), and 16, 18 (cancer)
Who is the Gardasil shot approved for?
Women 9-26; They get 3 doses
How does the Cervarix shot differ from Gardasil?
Women 10-25; Covers 16,18 (cancer) but also randos 31, 45
