Extrinsic allergic alveolitis Flashcards
1
Q
What is extrinsic allergic alveolitis also known as?
A
hypersensitivity pneumonitis
2
Q
What type of hypersensitivity reaction is thought to cause EAA?
A
immune-complex mediated (type III hypersensitivity)
**delayed hypersensitivity (type IV) is also thought to play a role in the chronic phase.
3
Q
Examples of EAA
A
- Bird fanciers’ lung:
- avian proteins from bird droppings - Farmers lung:
- spores of Saccharopolyspora rectivirgula from wet hay - Malt workers’ lung:
- Aspergillus clavatus - Mushroom workers’ lung: thermophilic actinomycetes*
4
Q
Acute presentation of EAA (occurring 4-8 hours post exposure)
A
- dyspnoea
- dry cough
- fever
5
Q
Chronic presentation of EAA (weeks-months after exposure)
A
- lethargy
- dyspnoea
- productive cough
- weight loss
6
Q
What investigations can confirm a diagnosis of EAA?
A
- imaging showing upper/mid-zone fibrosis
- bronchoalveolar lavage with lymphocytosis
- serologic assays for specific IgG antibodies
- bloods show NO eosinophilia
7
Q
Management of EAA
A
avoid precipitating factors
oral glucocorticoids
