Extraction Complications 1 Flashcards
when can extraction complications occur
immediately during operation
immediately post op
long term post op
name some peri-operative complications
difficult access
abnormal resistance
fracture tooth/root/bone
jaw fracture
maxillary antrum involvement
loss of tooth
soft tissue damage
haemorrhage
damage to nerves/vessels
what can result in difficulty in access
trismus
reduced aperture of mouth
crowded/mispositioned teeth
what is abnormal resistance due to
thick cortical bone
shape of roots
number of roots
hypercementosis
ankylosis
what could cause a tooth/root fracture
caries
alignment
size
root morphology
where does the alveolar bone usually fracture
canines/molars
in buccal plate
what jaw fractures more commonly
mandible
what could cause a jaw fracture
impacted wisdom tooth, large cyst or atrophic mandible
what do you do if a jaw fracture occurs
inform patient
post-op radiograph
refer
ensure analgesia
stabilise
antibiotic if delay
what is involvement of the maxillary antrum
OAF/OAC
loss of root into antrum
fractured tuberosity
how do you diagnose if there is involvement in the maxillary antrum
size of tooth
radiographic position of roots in relation to antrum
bone at trifurcation of roots
bubbling of blood
nose holding test
direct vision
good light and suction
blunt probe
what are the risk factors which can cause a maxillary antrum involvement
extraction of upper molars and premolars
close relationship of roots to sinus
last standing molars
large, bulbous roots
older patient
previous OAC
recurrent sinusitis
how do you manage involvement with maxillary antrum if it is small
inform patient
encourage clot
suture margins
antibiotic
post op instructions
how do you manage a large maxillary antrum involvement
close with buccal advancement flap
antibiotics and nose blowing instructions
how do you confirm if there is a root stuck in the antrum
confirm radiographically via OPT, occlusal or periapical
