- root compression
- cervical disc syndrome
- cervical stenosis
- lumbar disc syndrome
- cauda equina
- lumbar stenosis
- neurogenic claudication
what is cervical disc syndrome
- nucleus pulp herniates through the annulus (C6-7)
- pain in arm follows nerve root dist.
- LMN signs and Sx
Tx of cervical disc
conservative
- activity
surg
- anterior discectomy
what is cervical stenosis
chroinc disc degen. and assoc. facet arthropathy
- mech. neck pain, myelopathy and radiculopathy
Tx of cervical stenosis
- decompression and stabilization
- NAIDS, heat, ROM and strength
- surgery if severe
what is lumbar disc syndrome
herniated disc compressing nerve root BELOW level of disc
- initially back pain, then leg>back
Tx of lumbar disc
conservative
surg. only for cauda equina
what is cauda equina
compression or irritation of nerve roots below conus medullarus (L2)
- disc, fractures or tumors
4 features of cauda equina
- acute
- LMN weakness/paras
- autonomic
- bowel/bladder - sensory
- LBP
- saddle anasthesia
Tx of cauda
immed. surg. decompression
what is lumbar spinal stenosis
congenital narrowing of spinal canal combined with degen. changes
- gradually progressive back and leg pain made better by sitting
Tx of lumbar spinal stenois
- NSAID anal
- laminectomy if need be
what is neurogenic claudication
ischemia of lumbosacral nerve roots secondary to vascular compromise
- dermatomal pain with increased activity
what is main intradural intramedulary?
syringomelia (syrinx)
