Clinical features
- flexed posture of DIPJ
- unable to extend DIPJ
Management
- soft mallet - stack splint 8 weeks full time and 2 weeks high risk activities - BSSH guidelines
- bony mallet (reducible) - stack splint 6 weeks full time and 2 weeks high risk activities - BSSH guidelines
- bony mallet (volar subluxation) - closed reduction and axial K-wire across the DIPJ to achieve joint congruency
- open zone 1 extensor injury - washout/ debridement of wound/ DIPJ and repair of tendon
Chronic mallet
- 6 weeks stack splint
- no improvement DIPJ fusion
