What is best for someone suffering from Excited Delirium Syndrome
Fast termination of severe agitation
EDS is thought to be associated with what
The dysregulation of dopamine
What can increased dopamine do to the body
Aggression, paranoia, tachycardia, tachypnea, hyperthermia (hallmark symptom!)
Is associated with the use most commonly of what drug
Cocaine (blocks the re-uptake of dopamine)
Hyperthermia is caused by
Increased dopamine and stimulant effect = increase in metabolic activity which can cause hyperthermia
Sudden death in EDS can be caused by
Metabolic acidosis which can cause hyperkalemia and dysrhythmias, resulting in the potential for sudden death
Rhabdomyolysis can be caused by
Increased muscle activity and aggression exacerbation while attempting to restrain, increasing creatinine phosphokinase
Presentation (NOT A CRIME)
Pt is NAKED and sweating due to hyperthermia
Pt is violent against OBJECTS, in particular glass
Pt is TOUGH and basically a superhuman
ACUTE onset
CONFUSED
RESISTANT
INCOHERENT
MENTAL health conditions make you uncomfortable
EARLY backup is key for EMS
Why is less restraints early on is important for pt w EDS
Arrest is almost always fatal so restrain ASAP, restrain pt w as little as possible bc more will increase CBT and acidosis
What is the optimal drug use for EDS and why
Ketamine: quick acting, more predicable in uncontrolled environment
If ACP is unavailable what do you do
Transport ASAP!!
