In what order do you describe a skin condition?
Distribution - which area(s)
Configuration - how lesions arranged
Morphology - what lesions look like
What are the different configurations of rashes?
Discrete - well demarcated, individual lesions clearly seen
Coalescing - individual lesions present but starting to merge
Confluent - no normal skin
Grouped - individual lesions make up line (linear) or ring (annular)
What features of a rash’s morphology can be described?
Colour Size Raised or flat Border features Surface features (scale, crust) Use terminology (patch, plaque etc) Are all lesions the same
What are the possible differences in colour of a rash?
Purple, doesn’t blanch - purpura (small area - petechia)
Hyperpigmentation
Hypopigmentation
Erythema (will blanch)
What is macule?
Small <1cm flat lesion
What is a patch?
Larger >1cm flat lesion
What is a papule?
Small <0.5cm raised lesion
What is a nodule?
Larger >0.5cm raised lesion
What is the difference between macular and papular rash?
Macular is flat
Papular is raised
What is a maculopapular rash?
One that has flat and raised elements
What is a wheal?
A raised lesions that is compressible and has dermal swelling
What is plaque?
Lesion with a raised edge and a flatter surface >1cm
What is a vesicle?
<0.5cm fluid filled lesion
What is a bulla?
> 0.5cm fluid filled lesion
What is a cyst?
Nodule containing semi-solid material
