An ML5 is a medical certificate valid for inshore use. Depending on the category of area of use of my ship, that would be valid if we sailed within 60 Nm less then 24 meters, if we had to go offshore he would need to have an ENG1
Your new mate has arrived, he has an ML5 medical, his medical has expired but you need to sail today. What do you do?
If his medical is expired I would prefer not to go to sea. However, if he have a valid ENG1 medical unrestricted it can be extended for three months

I would not leave port, I am required to have the STCW certificates in date. I wold get another Chef if possible

A discharge book I required for all seafarers working onboard ship.
If he is a British citizen or working on a British flagged boat I can apply directly to the MCA
Who looks after the crew?
The Master, that is his role. Safety of life.
As the master we must insured that they have appropriate certifications, passports, that they are medicaly fit, must assured that food and water and accommodations are appropriate.
The master look for the crew, the passengers, the ship and the marine environment.

This must be kept on board your boat and is valid for 5 years.

-It is the limitations on the draught to which a ship may be loaded
The International Convention on Load Lines (CLL) was signed in London on 5 April 1966, amended by the 1988 Protocol and further revised in 2003.
Vessels operating at sea for non-pleasure purposes must under Merchant Shipping legislation hold a valid Load Line Certificate.
Alternative arrangements for small vessels have been made in the form of Codes of Practice. These vessels are issued with Small Commercial Vessel or Workboat Certificates in lieu of the Load Line Certificate or Load Line Exemption Certificate.
-A Document of Compliance (DOC) will be issued to a Company when the shore-side aspects of the SMS are fully compliant with the requirements of the ISM Code. The DOC is specific to the ship type(s) operated by the Company and for which the SMS is implemented at the time of audit.

Refuse to take part in a safety drill is dereliction of duty, not following a command = gross misconduct.
However, as the Master we have to take good care of the crew, so we have to know why.
Would need to talk with the stewardess. She could be bullied or harassed or simply tired.
I would have a look at her hours of work and rest.
I would check before initiate the hearing procedure.
I would conduct the drill, inform the crew that a member will be missing for the drill, and then do the whole procedural sequence

Being bullied = gross misconduct = have to go trough disciplinary hearing.

Behaviour towards others.
A person’s anti-social behaviour can be a nuisance to others on board. In extreme circumstances, it can also place the ship and the crew at risk of danger. Such behaviour includes but is not limited to excessive noise, abusive language, harassment, bullying, aggressive attitudes and offensive personal habits. Seafarers should also be considerate towards those who need to sleep whilst others are awake.
