severe and persistent substernal pain; sympathetic affect: cool and clammy, diaphoresis; parasympathetic affect: nausea and vomiting, weakness; mild fever; S4 (and possibly S3), may be systolic murmur or dyskinetic bulge; may have jugular venous distention or pulmonary rales
