Short head: Coracoid Process apex
Long head: supra glen old tubercle of the scapula.
Both: Attach to posterior area of the Radial Tuberosity

Action: Flexionof elbow, flexion of arm at GH joint.
Supraspinatus
Origin: medial 2/3 of supraspinus fossa
Insertion: superior facet of greater tubercle
Nerve: Suprascapular nerve C5/6
Action: stabilizer through abduction of shoulder joint.
Infraspinatus:
Origin: Medial 2/3 infraspinous fossa
Insertion: Middle facet of greater tubercle
Nerve: Suprascapular nerve C5/6
Action: Stabilizer + external rotation.
Teres Minor:
Origin: Upper 2/3 of lateral dorsal surface of scapula.
Insertion: Inferior facet of greater tubercle
Nerve: Axillary nerve C5/6
Action: Stabilizer,weak abduction and external rotation of Arm at GH joint.
Subscapularis:
Origin: Subscapular fossa (ventral surface of scapula.
Insertion: lesser tubercle of humerus
nerve: upper/lower subscapular nerve C5/6
Action: stabilizer, internal rotation of arm at GH joint
Latissimus dorsi:
Origin: T7-S5 spinous process and supraspinous ligaments. Lateral iliac crest, lower 3-4 ribs, inferior angle of scapula.
Insertion: interbicipital groove of humerus.
Nerve: thoracodorsal nerve: C6-C8
Action: extension, medial rotation, adduction, deep inspiration and forced expiration
Serratus Anterior:
Origin: upper 8-10 ribs near costal cartilage via fleshy digitation
Insertion: interdigitations run towards the medial border of the scapula.
Nerve: Long thoracic nerve C5-C7
Action: protraction of scapula, suspend scapula, pull inferior angle of scapula forward to assist in upward rotation of scap. Stabilization of scapula to prevent winging.
Flexor Carpi radialis:
Origin: medial epicondyle
Insertion: palmar surface base of 2nd metacarpal, slip on 3rd metacarpal
nerve: median nerve C6-C7
Action: flexion + radial deviation
Middle Trapezius:
Origin: Spinous process and supraspinous ligaments T1-T5
Insertion: medial acromial margin and superior lip of crest of scapular spine.
Nerve: Spinal accessory nerve + sensory branches ventral rami C3-C4
Action: scapular retraction, and stabilization
Brachioradialis:
Origin: proximal 2/3rd of lateral supracondylar ridge of humerus and anterior surface of lateral inter muscular septum
Insertion: radial styloid process
Nerve: radial nerve C5-C6
Action: Flexion of elbow (particularly in partial pronation) rapid elbow movements
Triceps Brachii
Origin: long head: infraglenoid tubercle of scapula. Lateral head: posterior surface of humerus superior to radial groove. Medial head: posterior surface of humerus inferior to radial groove.
Insertion: upper surface of olecranon process
Nerve: radial nerve (C6-C8)
Action: extension of elbow, extension of shoulder via long head.
Pronates teres:
Origin: humeral head: proximal medial epicondyle from common flexor tendon. Ulnar head: medial side of coronoid process of ulna
Insertion: lateral surface of radial shaft
Nerve: median nerve C6-C7
Action: pronation of the forearm in rapid forceful movement and weak elbow flexor
Supinator:
Origin: superficial tendinitis fibres and deep muscular from lateral epicondyle of humerus
Insertion: lateral surface of proximal third of radius, ant and post between anterior oblique line and fainter posterior oblique ridge, can have variations.
Nerve: posterior interosseous nerve (branch of radial nerve) C6-C7
Action: supination in slow movements
Deltoid:
Origin: anterior border and superior surface of lateral 1/3 clavicle. Lateral margin and superior portion of acromion. Lower edge of spine of scapula.
Insertion: deltoid tuberosity
Nerve: axillary C5/6
Action: anterior; flexion, medial rotation, vertical abduction. Middle: cervical abduction. Posterior: vertical abduction lateral rotation and extension.
Pectoralis Major:
Origin: Clavicular division: medial 1/2 clavical. Sternal division: anterior surface of sternum down to level of 6th and 7th costal cartilages, sternal end of first 7 ribs and aponeurosis external oblique.
Nerve: Medial lateral pectoral nerve C5-T1
Action: adduction and medial rotation of arm across chest.
Rhomboids:
Origin: Major: 2nd-5th thoracic vertebrae. Minor: C7-T1 vertebrae, lower nuchae ligament
Insertion: Major:medial border of scapular between root of spine and inferior angle. Minor: Medial end of spine of scapula
Nerve: Dorsal Scapular nerve C4-5
Action: scapular retraction
Muscles which attach to the Coracoid process:
Biceps short head.
Coracoidbrachialis: origin coracoid process, insertion; humeral shaft between triceps and brachialis. Nerve: musculocutaneus nerve C5-C7. Action flexion and horizontal adduction of arm
Pectoalis minor: origin: outer surface of 3rd to 5th ribs, near cartilages, insertion: coracoid process. Nerve: medial and lateral pectoral nerves C5-T1. Action: with SA draws scapular forward around chest wall. With levator and rhomboids rotates scapula, depressing the point of shoulder
Lower Trap:
origin: spinous process and supraspinous ligament from T6 to T12, insertion: aponeurosis at medial end of scapular spine and is attached to a tubercle on lateral apex.
Nerve: spinal part of accessory nerve, sensory branches from C3-4.
Action: scapular retraction
