What is the dosing for Acetaminophen in adults?
325mg PO Q4H PRN
MAX dose: 3-4g/day (liver dz = 2g/day)
What is the pediatric dosing for Acetaminophen?
10-15mg/kg PO Q4H PRN
Max dose of 75mg/kg/day
What is Acetaminophen usually used for?
mild pain and antipyretic
What are the common side effects of Acetaminophen?
Hepatotoxicity
What anticonvulsants are commonly used for neuropathic pain?
Gabapentin and Pregabalin
What is the dose for Gabapentin for pain control?
300mg PO day 1, then 300mg PO BID day 2 then 300mg PO TID day 3. Can continue to titrate until max of 3600mg/day is reached.
What is the dose for Pregabalin for neuropathic pain?
150mg PO as a single dose daily or 75mg PO BID
What type of pain are the anticonvulsants used for?
fibromyalgia, Neuropathy and post operative pain
What are the side effects of the anticonvulsants?
Sedation and dizziness
What are the SNRI’s used for pain treatment?
Duloxetine and Venlafaxine
What is the dose of duloxetine for pain control?
30mg PO daily
What is the dose of Venlafaxine?
37.5 - 75 mg PO daily
What type of pain are SNRI’s used to treat?
Fibromyalgia, Neuropathy, Depression
What are the side effects of SNRI’s?
Headache, sedation, weakness, nausea, HTN
How should the doses of the SNRI’s be adjusted if the CrCl is less than 30?
Avoid duloxetine, and decrease venlafaxine dose by 50%
What TCA can be used to treat pain?
Amitriptyline at 25mg PO daily
What do the TCA’s help to treat?
Neuropathy and Depression
What are the side effects of Amitryptiline?
anticholinergic side effects and sedation
What two opioids can be used to treat mild - moderate pain?
Codeine and Tramadol
What is the initial adult dosing for Codeine?
15-30 mg PO Q4H (Tablet or solution orally)

50mg by mouth every 4 hrs
