How is premature cervical dilation a cause of bleeding in 2nd trimester?
Cervix cant hold fetus to term.
Can treat with cervical cerclage (suture close)
If placenta previa, will patient have pain? Do we do vaginal exam?
No and no.
What are some causes of bleeding in 3rd trimester?
DIC, previa, abrupto, PIH (preg induced HTN)
What do we give for preeclampsia?
Mag sulfate.
Watch UO, RR. Slurred speech, DTR decrease, muscle weakness.
Toxic level is above 10
What does HELLP stand for?
Hemolysis
Elevated liver enzymes
Low platelets.
What is HELLP?
A major bleeding disorder. Often fatal.
What are some other 3rd trimester problems?
Preterm labor- 20-37 weeks
Postterm- >42 weeks.
PROM- probably from GBS. No vaginal exams
What are the danger signs of preeclampsia?
Visual disturbances, edema, HA, epigastric pain.
Random
Persistant vomiting— Mole Fluid discharge- previa, miscarriage Elevated temp- infection Abdominal pain-abruption Decrease is fetal movement- fetal distress.
