to be eligible for a pharmacy permit, a pharmacy must:
- be stationary (unless registered otherwise)
- be sufficiently secure and able to detect entry when pharmacy is closed
- be well lighted and ventilated and otherwise cleanly
- be equipped with sink and running hot and cold water, proper sewage, and refrigeration
- Be of sufficient size to allow for the practice of pharmacy
- have other equipment as required by the board to operate in compliance with state and federal law
When opening a pharmacy, a person must include what on application?

transferring prescriptions: upon request of a patient, a pharmacy SHALL transfer to another pharmacy a prescription for a patient (including C-II), that the pharmacy has received but not filled, unless:

when can a pharmacist refill a prescription?
can be done by a pharmacist without written, electronically transmitted, or oral authorization of a licensed practitioner (normally needed) IF (must meet all criteria):
- the pharmacist made every reasonable effort to obtain authorization to refill from the prescriber (reasonable effort)
- the pharmacist believes that failure to provide a refill would be detrimental to the patient’s health (detrimental to pt health)
- the original prescription authorized refill, but not all refills were dispensed or the prescription is expired (not all refills used/Rx expired)
- the prescription was originally filled at the pharmacy where the request for refill is received, or at an affiliate sharing a real-time, online database (refilling at OG pharmacy or sharing real time, online database)
