What is acute post-streptococcal glomerulonephritis (APSGN)?
Untreated strep throat that hits the kidneys
Most common childhood renal disease
Injury and inflammation to glomerular basement membrane
Most cases commonly occur between 4-12 years
strep → untreated → infiltrates glomerular membrane
Strain group a hemolytic strep hits the kidneys
What are the signs and symptoms of strep?
Hypertension
Edema
Hypervolemia
Increased respiratory work, crackles
Fever, headache
Pale, lethargic, irritable
Vomiting, anorexia, abdominal pain
Dark/tea colored urine
Decreased urine output
How is strep diagnosed?
Throat culture
Urinalysis
Check rate of inflammatory response, if an active infection is happening, and renal function
Complications of strep
Severe hypertension
Hypertensive encephalopathy
Seizures
Cardiac decompensation from hypervolemia
Acute renal failure
Treatment for strep
Antibiotics ONLY if throat culture positive
Supportive
Treat symptoms (antihypertensives)
Nursing management for strep
BP
Fluid balance
Behavior
Regular diet, no added salt
Fluid restrictions
Activity restrictions if hypertensive or cardiac s/s
Respiratory distress (crackles) if fluid not removed
What is the parent education for strep?
Prevention of infection
Medications
Adequate rest
