Electronic controlled substance transfers
can be transferred to another pharmacy even if not filled before if:
- allowed by state law
- the prescription was electronically prescribed
- if communicated between to RPh
- if prescription is maintained in an electronic format
non-electronic controlled substance transfers
limited to C3,C4,C5
must be filled at least once before being eligible to be transferred
controlled substance transfer documentation
write “VOID” on face of invalidated prescription, or for electronic prescription that has been transferred
record the name, address, DEA of receiving pharmacy
record date of transfer and name of the transferring pharmacist
receiving pharmacist must note
- original date of issue of prescription
- original number of refills authorized
- date of original dispensing
- number of refills remaining
- transferring pharmacy name, address, DEA, and prescription #
- name of transferring pharmacist
- original pharmacy name, address, DEA #, and prescription #
substance use disorder
problematic pattern of using a substance that results in impairment of daily life or noticeable distress
detoxification
dispensing a narcotic drug in decreasing doses to bring a patient to a drug free state
short-term detox: 30 days or less
long-term detox: 30 - 180 days
maintenance
maintaining patient on a stable dose of a narcotic drug for more than 180 days
substance use disorder treatment
a prescription may not be issued for detox treatment or maintenance unless it is C3,C4,C5 narcotic approved by FDA
- practitioner registered with DEA as narcotic or opioid treatment program
- partitioner dispensing from their office supply
- hospitalized patient
- prescription for direct administration
SUD: practitioner registered as an NTP/OTP
may adminster or dispense (not prescribe) a narcotic listed in any schedule for maintanence/detoxification therapy
reffered to as methadone clinics
methadone is not approved for detox or maintenance
SUD: practitioner dispensing from office supply
limited to a three-day supply of medication to be dispensed
no extensions on three days, it is one and done
SUD: hospitalized patients
patient must be hospitalized due to an issue outside of substance use disorder
goal is to not interrupt their treatment
SUD: prescription for direct administration
the controlled substance must be administered by injection or implantation
it must be administered to the patient named in the prescription within 14 days of receiving the product from the pharmacy
Distributing a controlled substance to another registrant
registrant must be appropriately registered with the DEA
The pharmacy records distribution and receiving registrant as required
C1 and C2s require DEA form 222 completed by both parties
Dosage units does not exceed 5% of total dosage units all CS distributed and dispensed in the same calendar year
distributing a controlled substance to a reverse distributor
- the pharmacy contacts the reverse distributor with a list needing disposal
- the reverse distributor creates an invoice for destroyed products (C1/C2s need DEA 222)
- Pharmacy completes suppliers’ requirements and sends drugs to reverse distributor
- Reverse distributor fills out DEA form 41
four methods of controlled substance disposal
on-site destruction: two employees handle destruction, which makes substance non-retrievable
delivery to a reverse distributor
sending product back to distributor/manufacturer/warehouse: only for recalls
requesting assistance from local DEA office: only done by practitioners
DEA form 41
must be completed for controlled substance destruction
once completed, maintained with other CS forms and does not need to be sent to DEA
theft or significant loss
must be reported to DEA field office and board of pharmacy
DEA form 106 must be filed within 45 days of discovery
all theft (1 tab to 1,000 tabs) must be reported
only significant losses must be reported
DEA form 106
for controlled substance theft or significant loss requires:
date of occurence/discovery
theft or significant loss recap
name, dosage form, strength, quantity of all controlled substances lost/stolen
executed forms should be sent to DEA filed office and board of pharmacy
