Difference between Primary and secondary OA
Primary- MC form, adults, no major trauma
Secondary- previous injury or deformity
OA occurs where in men and women
Females- knees and hands
Males- hips
What is chondromalacia
When matrix cracks and softens, exposes subchondral bone
What is eburnation
Bony sclerosis and osteophytosis in OA
What is a subchondral cyst
When fluid is in the bone due to OA
Possible joint mice= painful locking
What is Heberden’s nodes?
Occurs in the hands due to OA at the DIP joints
Bouchard node occurs lover in the finger
What Rheumatoid arthritis
Chronic autoimmune disease, CD4 +T cells, cytokines
+HLA-DRB1
Tests for RA
Anti cyclic Citrullinated peptides 70% of cases smoking causes false positives
RF factor test IgM antibodies 80% accurate
RA is most common where
Small joints hands/ wrist
30% of patients have Atlanto-axial instability
Pannus
RA is more common in who?
Females 3-5x more likely ages 35-50
Ulnar deviation is symptom
What is felty syndrome?
Combo of RA, splenomegaly, neutropenia
Presents as classic RA
What’s neutropenia
Risk for recurrent infections
What’s Caplin syndrome
Comb of RA, and Pneumoconiosis (coal)
Presents as RA
May mimic asthma, VERY RARE
Juvenile Rheumatoid arthritis is
Less than 16 yrs old MC 1-6.
Oligoarticular- few joints
Polyarticular- many joints
Still’s disease- JRA+spiking fever, rash and pericarditis
MC at large joints knees and hips
Girls 2-3x more at risk
Whats Scheuermann’s disease
Aka juvenile kyphosis of thoracolumbar spine
MC males, 13-16 familial
