chronic conditions
- one in three adults live with more than one chronic condition
- longevity and advances in healthcare contribute the increase in chronic conditions
- symptoms persist over time, months, years or a lifetime
- arthritis, diabetes, clinical depression, or cardiovascular disease
disability
- an umbrella term for impairments, activity limitations and participation restrictions
- people can have chronic conditions and not be disabled
chronic conditions and disabilities
- care for individuals with chronic conditions and disabilities may be provided throughout the lifespan
- parents and providers (care team) must work together to determine the most caring response
- may require early intervention services for infants
- quality of life decisions must be determined by the care team
who makes health care decisions for a competent disabled patient who has a health care proxy and why
the patient is disabled but competent so the patient will make the decisions -> final

a patient who is diagnosed with bipolar depression signed a do not resuscitate (DNR). three months later, the patient goes into cardiac arrest and is unresponsive. the patient is transported to a local hospital where he was previously treated. the patients spouse is the designated health care proxy and disagrees with the DNR

health disparity
- a health difference on the basis of one or more health outcomes that adversely affects disadvantaged populations
- race, ethnicity, socioeconomic status, sexuality, sexual orientation, and identity, gender, skin color, and disability are all recognized as fostering inequities in healthcare and health outcomes.
