Pt has severe pleuritic retrosternal CP that is worse when taking deep breaths or swallowing. PE shows crepitus and Hamman’s sign (mediastinal “crackle/crunch” with every heart beat)

Upper Endoscopy
Test of choice for Boerhaave syndroem?
Esophagram + Gastrografin Contrast
if inconclusive -> get Chest CT
Trmnt for Mallory-Weiss Syndrome?
supportive
Most cases stop bleeding w/o trmnt
PPIs may help promote healing
Trmnt for Boerhaave Syndrome?
NPO
IV ABX
IV PPIs - Esmeprazole (Nexium)
Sx Consult (large or severe ones need Sx repair)
