What causes epiglottitis?
A
Haemophilis influenza B infection usually
Causes inflammation of the epiglottis - can completely obscure the airway so it is a life-threatening emergency

Presentation suggesting epiglottitis
A
Sore throat and stridor Drooling Tripod position - sat forward with hands on knees High fever Difficulty or unable to swallow/speak

Investigations
A
DON’T EXAMINE THE AIRWAY
If suspect epiglottis best not to do any investigations and just call anaesthetist

Management
A
Don’t do anything to distress the patient - could prompt closure of the airway
Call senior paediatrician and anaesthetist - to secure the airway - child may need intubation
Once airway is secure:
- IV antibiotics e.g. ceftriaxone
- Steroids e.g. dex

Complications of epiglottitis
A
Epiglottic abscess - also a life threatening emergency
Death
