What are the two most common causes of an epidural abscess?
A
Hematogenous spread (50%) Spread from diskitis (33%)

How does an epidural abscess present?
A
Pain, fever (50%)
Elevated WBC, ESR and CRP
Neurologic symptoms (33%)
MRI with contrast is diagnostic

What is the treatment for epidural abscess?
A
No neuro signs can treat with IV abx
If neuro signs or evidence of compression needs decompression; laminectomy (posterior), anterior debridement with strut graft (anterior) followed by 2-6wks of abx
Can instrument spine if unstable, even with infection
